Home Health Care & Hospice Services

Home Health Care Description: A home health agency that provides skilled nursing care and rehabilitation services, including the administration of chemotherapy and other treatments, for people unable to leave home. The nurses can also instruct family members on how to give care. Referral can come from doctors, nurses, social workers, discharge planners and family members - you can even refer yourself. However, insurance companies require a doctor referral. Only skilled home health care is covered in part by Medicare and Medicaid. Personal care services are available through private duty nursing agencies.

Hospice Care Description: Hospice services are available to people who can no longer benefit from curative treatment and typically have a life expectancy of six months or less. Hospices provide sensitive support, managing pain and other symptoms so that patients can spend their last days with dignity and quality at home or in a home-like setting. A team of trained professionals provide services to the patient and the entire family, including them in the decision-making process. Referrals can be made by a doctor or other health-care professional, clergy, family member or friend. While each hospice program has its own admission criteria, hospice is a covered benefit under many private insurance plans and it is a covered Medicare and Medicaid benefit.

Johns Hopkins Home Health is a Medicare/Medicaid certified, non-profit, hospital-based agency that provides health services to ease the transition from health care facility to home. Johns Hopkins Home Health nurses and therapists work closely with physicians to develop the best course of individualized, quality home treatment to help patients regain independence. Services include nursing (medical/surgical, mental health, wound/ostomy care); physical, speech and occupational therapies; medical social work; diet counseling and personal care.

VNA of Maryland provides skilled nursing, physical therapy, occupational therapy, respiratory therapy, speech and language therapy, durable medical equipment, infusion therapy, nutrition consultation, enterostomal therapy, private duty care and hospice.
